What the form asks,
and why.
Your answers reach the technician ahead of time, which improves the chance the right part is already on board. None of it commits you: the price is given after the diagnosis.

The three steps above are the booking. These four are what happens once a technician is in your kitchen.
What repairs costFind the cause
The technician runs the machine and watches what it actually does, then rules out a clogged filter, a kinked hose or a tripped setting before looking at parts.
Price it first
You hear what the repair involves and what it will cost before anything is touched. If a new dishwasher makes more sense, you hear that as well.
The part
Frequent parts ride on the van. Anything tied to one model is ordered by its model number, so giving it at booking saves a day or more.
Fix and run a cycle
The repair is made and a cycle is run with you there, so the fill, wash and drain are seen working rather than taken on trust.

What a code meansDo not keep resetting it
Repeated resets can clear the stored code the technician would read. Leave it off only if it is leaking, tripping the breaker or smells hot.
Note any code
If the panel shows a code, write it down character for character. Identical codes can mean different things on different makes.
Empty and clear it
Take the dishes out and clear the cabinet under the sink. Note when it started and what came first: a new detergent, a plumbing job, a power outage.
